New Patient Forms

If this is your first consultation with any of our doctors, please complete the relevant forms before your appointment. This helps your doctor prepare and means we can make the most of your time together.

Start with the registration form, then complete the intake form based on the biological system relevant to your consultation.

A note on the intake forms: These questionnaires ask about biological systems and are not related to your gender identity. If you don't identify with either form, or feel they don't apply to you, you're welcome to skip them — please just let us know so we can support you in the way that works best.

Blood Test Results

We may email you a request for blood tests. It is by no means compulsory, but it is strongly advised for certain conditions. Please try to have the tests done before your consultation.
